Safety Culture and Disastrous Decisions

We are writing to express our concerns about the article “Prevent Disastrous Decisions” in the July 2020 issue (pp. 47–53). While decision-making plays a role in preventing catastrophic chemical plant accidents, we feel the article does not reflect fundamental advances in process safety that have taken place over the last several decades, including industry’s increasing understanding of organizational safety culture as a determinant of success in achieving an organization’s process safety goals. Our concerns address three primary subjects.
