Until 2010, personnel safety efforts concentrated on preventing minor incidents and hoped for a trickle up effect. This session shares research that challenges this long-held practice and offers a new approach. We will explain why organizations must focus on preventing the most serious incidents first and discuss applications to process safety. We will explore implications such as safety leadership, improvement strategy, leading indicators, and human performance.
