Equipment Identification - Strategies & Effective Practices to Manage and Mitigate Hazards

Proper equipment tagging

  • Periodically complete a site survey to ensure that the Process and Instrumentation Diagram (P&ID, also known as a flowsheet) accurately depicts plant equipment, valves and instruments as installed in the facility.

Equipment Identification - Potential Hazardous Consequences

Personal Injury (Safety / Human Health)

  • Improper tagging  or identification of equipment or piping may lead to release of hazardous material resulting in personnel exposure to fire and explosion, radiation or toxic vapors.

Equipment Identification - Need / Call to Action

Incidents Continue to Happen

“Release of toxic chlorine gas during a routine delivery activity due to the inadvertent mixing of sulphuric acid and sodium hypochlorite, injuring 140 people in a nearby community. Improper Equipment Identification of piping, specifically chemical lines containing incompatible materials, resulted in a release of chlorine gas. Investigation recommends that attention be paid for design, identification and operation of the equipment.”

Equipment Identification - Fundamental Intent

To identify and manage situations where incorrect Equipment Identification may pose significant hazards during performance any job or during any incident in plant areas. Such hazardous situations may result in fires or explosions that could result in personal injuries, property damage, community impacts, environmental impacts, or business impacts.

Example activities that would require use of an Equipment Identification SWP:
