2019 Food Innovation and Engineering (FOODIE) Conference

The Food Innovation & Engineering Conference (FOODIE), hosted by AIChE’s Food, Pharmaceutical & Bioengineering Division (Division 15), will offer a chance for people from multiple disciplines (including food engineers, neurobiologists, physiologists, members of supply chain, and more, as well as chefs, restauranteurs, and foodies alike) to network and share ideas to improve the food world.

The following losses occurred due to inadequate Permit to Work system management or control.  In all cases there were other factors also involved.

  • Non-routine work increases process risk and can directly lead to conditions that make a catastrophic accident much more likely.
  • Safe work practices are critical elements in the management of non-routine work.
  • Safe work practices include an integrated management system of policies, procedures and permits that help protect workers from hazards and prevent the sudden release of process materials or energy during non-routine work activities.

Conducting initial field inspection

  • Ensure that the equipment that will be worked on is identified/tagged prior to issuing the SWP. 
  • Confirm that the location is safe and does not require additional measures to control hazards. 
    • Safe access is available for the work crew 
    • Area is safe to move objects required for execution of the job 
    • Escape routes are clear

  • An energy release that occurs after opening the wrong equipment due to improper inspection of work site, resulting in damage to the assets.

Incidents Continue to Happen 

The Lloyd’s Market Association Oil, Gas and Petroleum Loss Analysis provided the first three examples of incidents resulting from the failure of the Permit to Work system.    

Fertilizer Plant Fire - Following the completion of welding work by contractors, a fire spread due to the presence of combustible materials in the area including cable trays and conveyor belts. 

Field Review of Permits - Fundamental Intent

To improve awareness among permit to work roles of the importance of field visits for identifying situational and human factors which might otherwise be missed during task risk assessments.  Not recognizing these factors before issuing permit might pose significant hazards during performing the planned tasks.
