The Method of Reviewing HAZOP Worksheet with Risk Evaluation

This report shows the method of reviewing HAZOP worksheet with risk evaluation in Idemitsu Kosan Co.,Ltd Hokkaido Refinery. We have studied P&ID by HAZOP owing to identifying and removing process safety hazards since 2003. The incident in 2019 was happened that identified in HAZOP study as a hazard. We identified that incident as the process safety hazard in HAZOP study, but the risk evaluation might not have been right. This is why we considered how to review HAZOP worksheet with headquarters’ expert engineers. As the result, we developed the method of reviewing HAZOP worksheet with risk evaluation. Our process safety team used the method and reviewed the HAZOP worksheet. We found the most of safe guard well and some of them need strengthen.
