Dr David Embrey is the founder and Managing Director of Human Reliability Ltd. He holds BSc and PhD degrees in physics and applied psychology from Birmingham and Aston universities. After an academic career at these Universities, he founded Human Reliability Associates in 1981. Subsequently he carried out research into human reliability assessment techniques for the US Nuclear Regulatory Commission, with Brookhaven and Oak Ridge National Laboratories and worked on consultancy and research projects in a wide range of industries, including chemical processing, aviation, air traffic control, nuclear power generation, rail and marine transport, and manned space flight. He is the primary author of the CCPS textbook 'Guidelines for Preventing Human Error in Process Safety'.
