Possible Work Flow







           Permit Authority — Permit Issuer: Area Operator

           This is typically the Area Operator whose responsibilities include:
           g  Determine confined space hazards by reviewing current drawings, P&IDs, specifications, and by a physical inspection
             of the work site. Engage with Engineering and Technical resources as required to identify hazards and safeguards.
           g  Developing a Confined Space Entry plan and controls/safeguards.
           g  Verifying all control measures are in place before work starts and that the worksite is safe for the work activities
             specified in the work permit.
           g  Ensure proper training and qualifications of confined space entrants
           g  Ensure proper training and qualifications of Entry Attendant

           g  Verify that the SWP requirements for associated SWPs (such as Energy Isolation) have been properly implemented
             prior to Confined Space Entry.
           g  Confirm that atmospheric monitoring requirements have been met and that ventilation requirements have been
             properly implemented prior to Confined Space Entry.
           g  Confirm that the required safety equipment for entry, exit and rescue is in place and in good working condition.
           g  Ensuring that the rescue team members are qualified and ready to execute a rescue. Confirm that all entrants and
             Entry Attendant are trained on the rescue plan.
           g  Provide means of communication for entrants, Entry Attendant, control room and rescuers.
           g  Conducting a “Toolbox talk” with all assembled work party members. Ensuring they understand the work instructions
             and the conditions of the Permit. Pointing out specific hazards and identifying how they will be managed for the
             duration of the work activity. Explaining contingency plans in the event of an emergency. Ensuring all work party
             members sign onto the work permit.



           Entrant — Permit Receiver: Contractor, Operator, or Other Person

           Is the responsible person for the activity being carried out under the authority of the permit. Is a competent person
           who is appropriately trained, has demonstrated practical skills, and is appointed by management to participate in a
           Confined Space Entry. This is typically the person whose responsibilities include:

           g   Reviewing and understanding the requirements of the permit. Receiving instruction from the Permit Issuer
             on hazards and safeguards/controls in place.
           g   Visiting the worksite and identifying any hazards which need to be properly controlled during the work task.
             This activity should be done as a joint walkthrough with the Permit Issuer.
           g   Participating in any risk assessment activity associated with the work task preparations.
           g   Reviewing and understanding the rescue plan and communications protocols related to the Confined Space Entry.
           g   Signing for the permit when issued and ensuring that all requirements and instructions of the permit are followed.

           g   Managing the conduct of the Confined Space Entry in accordance with the work order and the permit.
           g   In the event of an emergency or a general alarm, ceases work immediately, ensures the work area is made safe,
             exits from the confined space, and proceeds to their muster point.

           g   Ensuring the worksite is maintained in a safe and tidy manner during the work activities and that when the work
             is complete, the site is cleared of all tools and thoroughly cleaned before the work permit is signed off and returned
             to the control point.
