(54f) Best Practices for PHA Revalidation

Process hazard analyses (PHAs) must be updated and revalidated every 5 years or sooner. PHA Revalidation is entirely different than a baseline or original PHA. The original textbook from CCPS on Revalidating PHAs was issued in 2001. This paper describes the current best practice approaches to revalidation and explains when to use each approach. The approaches are described in detailed flowcharts and checklist for decisions making and quality control are provided, along with examples of completed documentation. This paper is based on completion of more than 300 Revalidations.
