(115a) A Study of Catastrophic Incident Warning Signs to Prevent Incidents in the Yeosu Chemical Complex

The hazard warning signs are early indicators that something is wrong or about to go wrong. We can prevent the incident and loss event, if we recognize and adequately measures on these hazard waring sings in a timely manner. If there are no apparent consequences from the deviation during process operation and maintenance, we accept those new practices and substitute the original practice. If we look and act the process safety risk factors cause the incident in the refinery and chemical plant, major incident can be prevented through eliminated or decreased risk element. in this study, a survey for the catastrophic incident warning signs are conducted to look specifically potential hazard element of refinery and chemical plant in the Yeosu chemical complex. Major incidents warning sign self-assessment checklist which was published by CCPS “Recognizing catastrophic incident warning signs in the process industry” was collect the hazard element caused incident occurred in the process industry through investigation was used for survey. Through this survey, the major incident warning sing in the process industry was founded by process safety manage element, each company and facility can use those signs for incident prevention activity. In addition, proposed what activities required to eliminate root cause of those warning signs.
