Driving Continued Improvement in Risk Assessments for Operating Facilities. Risk assessments for an operating facility, such as HAZOPs, LOPAs and QRAs, are typically part of an overall risk management program that is maintained and updated throughout the life of the facility. Over time, the goal should be continued improvement to the risk assessment, which should ultimately lead to improved safety in the facility. The types of activities that can drive continued improvement include study revalidations, periodic audits to validate effective implementation of management systems driven by the risk assessments, and quality improvements to the risk assessment and evaluation work processes themselves. This session solicits papers that describe the novel approaches taken and the results obtained toward driving continued improvements to risk assessments and the management systems that tie into those assessments at a facility, business unit, geographic or global level.
