Process Safety Beacon: Plugged Flame Arrester Causes Explosion

In Safety Alert No. 290 (Oct. 2010), the U.S. Dept. of the Interior’s Bureau of Ocean Energy Management (BOEM) explains an incident in which an explosion ruptured two oil tanks on an offshore platform. The explosion caused about $500,000 worth of damage and approximately 1,200 gal (4.5 m3) of oil to spill into the sea.
