CEP: Process Safety Beacon - Why Can't I Open that Valve?

In April 2004, an explosion and fire at a polyvinyl chloride (PVC) plant killed five people and severely injured three others. The explosion and fire destroyed most of the reactor building and an adjacent warehouse. Because smoke from the fire drifted over the community, authorities ordered an evacuation. The plant was never rebuilt.
